I have had a lot of twins. But dream plane
Large(10 ft +) B17 or B29. this is my test bed for engine outs
mixing etc....
4 engines enough of a hassle.
But I agree after I get bored maybe add one to the nose.
The designer of the 4-star Bruce thorpe told my buddy
"Fobus" the same when we asked him about making a
4*10 4 star.

Five people to start the engines - one in front of each wing and one behind, the fifth man held the tail.
The two in front would crank, while the two behind would move the glow clips, and after all were running top all eight tanks from the outer engines in order to the inner. This was in hopes the outboard engines would quit first, and then progressively the engines would stop working their way to the two inboard being the last engines running. It worked pretty well.
With all eight turning and burning the plane was set in the water, it promptly accelerated into flight, climbing regally. After a few turns around the field, climbing with all running, an engine or two would quit, and as they quit the climb would become a cruise in level flight, then with a few more shut down a descent. Finally with only two or three engines running a majestic approach and splash down. And if it was timed just right, the last two engines would quit just as the plane came to the dock.
Quite an airplane.

Wasn't my plane, I only saw it once. But I was impressed.
Throttle management wa a non-issue - there wasn't any. All engines ran flat out until out of fuel.
