What type of radio are you running? Are your batteries good and charged? Low batteries will cause a weak signal which could cause this. It's highly unlikely that a loose screw is causing your fail safe to wig out.

I do not have first hand experience, but I have also heard that an external fail safe can be a demon all on its own.

To tell you the truth I hate failsafes because they mess up the electronics. A loose screw would have no effect on the failsafe. Check your batts and your rx antenna and make sure its not cut off. Afterwards take out the failsafe and put the truck on blocks. Then run it so the wheels can move freely. Walk as far as you can go until you cant get a signal or it starts glitching. If its still glitching without the failsafe then you might have a problem with your reciever or you need to change your crystals because of maybe phone lines or power lines. This happened with my stock radio. If it dosnt glitch then throw away the failsafe or send it back to the company if its under warranty.
